I understand that basketball training, strength and conditioning, movement instruction, film study, and live or recorded online coaching involve risks, including falls, collisions, overexertion, equipment failure, property damage, serious injury, illness, disability, or death. I voluntarily choose to participate and accept all known and unknown risks.
I confirm that the participant is physically able to participate and will stop immediately if pain, dizziness, illness, unsafe conditions, or equipment problems occur. I am responsible for obtaining medical clearance when appropriate, maintaining a safe training area, using suitable equipment, supervising minors, and paying any medical expenses.
To the fullest extent permitted by Georgia law, I release and hold harmless Lazenby Player Development Hoops Academy, Coach Corey Lazenby, and their representatives from claims arising from ordinary negligence connected with participation. This release does not apply where prohibited by law or to gross negligence or intentional misconduct.
I authorize reasonable emergency assistance when necessary, understanding that no particular medical outcome is guaranteed. Online coaching cannot inspect or control the participant’s location, surface, equipment, supervision, or internet connection.
If the participant is under 18, I certify that I am the parent or legal guardian, consent to participation, accept this agreement for myself and the minor to the extent permitted by law, and will provide appropriate adult supervision.
This agreement is governed by Georgia law. If any provision is unenforceable, the remaining provisions remain effective. My typed legal name and required checkbox on the training form are intended as my electronic signature and agreement. The submitted request will record the signer’s name, acceptance date, agreement link, and source for LPD’s business records.
